Jerry Campbell, FNP-BC:
I am a board-certified Family Nurse Practitioner, certified by the American Nurses Credentialing Center (ANCC), earning my Master of Science in Nursing from Morehead State University. I have experienced providing primary care with a focus on family health, chronic disease management, and preventive care across the lifespan. It is especially meaningful to me to serve the small-town community where I grew up and care for patients and families in the place I call home. My goal is to provide thoughtful, evidence-based care while making sure patients feel heard, respected, and actively involved in their treatment decisions. She serves in the main Jackson facility as well as in Hazard and Prestonsburg.

Melody Brewer, MSN, APRN, FNP-C, is a board-certified Family Nurse Practitioner dedicated to providing compassionate, patient-centered care to individuals and families. She earned her Bachelor of Science in Nursing from Morehead State University and later completed her Master of Science in Nursing with a Family Nurse Practitioner concentration from Frontier Nursing University. She is board certified through the American Association of Nurse Practitioners (AANP). Melody provides care for patients of all ages and enjoys helping individuals and families manage both acute and chronic health conditions. She is committed to helping patients achieve and maintain optimal health through preventive care, chronic disease management, and patient education. She believes in building strong relationships with her patients and partnering with them to develop personalized care plans that support long-term health and wellness. Melody is available for both Telehealth visits as well as in person visits. Outside of the clinic, Melody is an avid crafter who enjoys woodworking, DIY home improvement projects, and scrapbooking. She also enjoys spending time with her children and staying active in her community.

Maddy is a native and lifetime resident of Breathitt County. She graduated from Eastern Kentucky University in 2022 as a Family Nurse Practitioner. I am board certified by both ANCC and AANP. She loves working with patients of all ages for both acute and chronic problems. She also has her certification to complete DOT physicals. Maddy’s main focus/specialty is primary care and chronic disease management. Maddy is available for both Telehealth and in person visits. On her time off from work she is a devoted mother and wife. She enjoys gardening, traveling & spending time with family.

I am Tiffany White, APRN-BC, a Family Nurse Practitioner certified through ANCC and a recent graduate of Northern Kentucky University. I have been a registered nurse for 8 years, with diverse experience in patient care across multiple clinical settings. My clinical focus includes primary care, acute illness management, and chronic disease management across the lifespan. I am committed to providing compassionate, patient-centered care, ensuring each patient feels heard, respected, and involved in their healthcare decisions. Patients can expect thorough evaluations, clear communication, and evidence-based treatment plans tailored to their individual needs. My area of focus and specialty is Primary Care, Family Medicine, Chronic Disease Management.

I am a board-certified mental health nurse practitioner with 4 years’ experience. I have been a nurse for 18 years. I specialize in mental health disorders, including depression, anxiety, mood disorders, and ADHD, across the lifespan starting at age 6. I believe in providing compassionate, patient-centered care. I will work with each patient individually to improve their mental health and overall well-being. I specialize and focus on Behavioral Health: depression, anxiety, mood disorders, personality disorders, conduct disorders, and ADHD. In addition, I perform both Telehealth and in clinic visits.
I am a dedicated Family Nurse Practitioner with additional experience in substance use disorder care. I earned my education from Vanderbilt University, where I developed a strong clinical foundation and a commitment to evidence-based practice. My professional passion lies in delivering compassionate, patient-centered care that meets individuals where they are, supporting both their physical and behavioral health needs. I strive to build trusting relationships with my patients and empower them on their journey toward improved health and well-being. I focus and specialize in Primary Care, MAT.

I am a Chris Jackson, MSN, APRN, FNP-C, a life-long resident of Breathitt County. I obtained my Master of Science in Nursing from South University. I am a board certified by the American Academy of Nurse Practitioners. I strive to provide compassionate patient-centered care for chronic and acute disease processes. My area of focus is in primary care and chronic disease management. Being able to provide care for the people of the place I call home is very important to me and I strongly believe in working with people to determine the best plan of care for them as an individual.

I am a Licensed Professional Clinical Counselor (LPCC) dedicated to supporting individuals through life’s challenges, including anxiety, depression, trauma, and substance use recovery. I have experience working with clients in medication-assisted treatment (MAT) and utilize evidence-based approaches such as Cognitive Behavioral Therapy and Motivational Interviewing. I believe in meeting each client where they are and building a strong, trusting therapeutic relationship. My goal is to create a safe, respectful space where patients feel comfortable, supported, and actively involved in their care. My specialty/area of focus is Anxiety, Depression, Trauma informed care, and Substance use recovery. I offer services both Telehealth and In-Person.

I earned my bachelor’s and master’s degrees and completed my clinical training at Lindsey Wilson College School of Professional Counseling. Over the past seven years, I’ve had the privilege of working with individuals facing substance use, co-occurring mental health concerns, and a variety of life challenges. I work with adults and adolescents who are navigating anxiety, depression, trauma, substance use, mood disorders, and relationship difficulties. I use an integrative approach that blends Cognitive Behavioral Therapy (CBT), Person-Centered Therapy, and Reality Therapy to support meaningful and lasting change. I believe therapy should feel like a safe, supportive space where you can be heard without judgment. I take a compassionate, collaborative approach, helping you better understand how your thoughts, emotions, and behaviors are interconnected. Together, we’ll build practical coping skills and strategies to help you move forward with confidence. Each session is tailored to your individual needs and goals. My name is Kimberly Smith and I am a Licensed Professional Clinical Counselor Supervisor and a Certified Clinical Anxiety Treatment Professional. I graduated with a Master's of Education in Human Services and Counseling from Lindsey Wilson College. I have worked as a counselor for 10 years treating co-occurring mental health and substance abuse disorders. I provide compassionate, evidence-based counseling services for kids ages 6-17 and adults. My goal is to tailor interventions to meet each client's developmental level, and unique needs, incorporating techniques from Cognitive Behavioral Therapy, Reality Therapy, Motivational Interviewing, and Client Centered practices. My work focuses on helping clients manage anxiety, depression, behavioral difficulties, improve self-esteem, and addiction. I personally believe client's have to be met where they are and that growth is possible at any stage of life, and I strive to help each client learn to cope and increase confidence to be their best self.

My main area of focus/specialty is Behavioral Health, Substance Abuse, Anxiety, Depression. I mainly do Telehealth. The types of therapy I offer are Individual therapy, Substance use counseling, Cognitive Behavioral Therapy (CBT), and Medication-Assisted treatment support.
