All CDL/DOT physicals are performed by a DOT‑certified provider and include the required components:
Blood pressure screening
Vision screening
Urinalysis
Physical exam
Before Your Appointment: What You Need to Know
Urinalysis
You will be required to provide a urine specimen. Please drink plenty of water before your appointment.
Blood Pressure
Avoid smoking for at least one hour before your exam, as it can raise your blood pressure. To pass, your blood pressure must be below 140/90.
Heart Disease
If you have a history of heart attack, stents, angioplasty, or angina, please bring a clearance letter from your cardiologist stating you can safely operate a commercial motor vehicle. You may also need to provide a recent exercise stress test and/or echocardiogram (within the last two years).
Medications
Take all your regular medications on the day of your appointment.
Diabetes
Bring a copy of your A1C lab results from your primary care provider, completed within the last three months.
Diabetes Treated With Insulin
Bring the completed Insulin‑Treated Diabetes Mellitus Assessment Form (MCSA‑5870) from your treating health care provider.
Sleep Apnea
Bring a CPAP compliance report from your CPAP provider or sleep specialist. The report must show:
At least 4 hours of nightly use
70% or more of nights used
Usage within the 30 days prior to your exam
Stroke, TIA, Seizure, or Brain Injury
Many individuals with these neurological conditions may not qualify for a DOT Medical Certificate. You may bring a neurologist’s statement indicating you can safely operate a commercial motor vehicle. This will be considered, but does not guarantee approval.
